Covid-19 Patient Information



We are screening for respiratory symptoms and Covid-19 prior to your appointment. If you answer ‘Yes’ to any of the following, please call us on 01844 215404 to discuss this with a member of staff before attending for your appointment.


  • Do you have any of the following symptoms:

    • A high temperature or fever?

    • A new, continuous cough?

    • A loss or alteration to taste or smell?

    • A runny nose?

    • A sore throat?

    • Difficulty breathing or shortness of breath?

  • Have you or a member of your household/family been advised to isolate by any NHS organisation in the last 10 days?

  • Are you or any member of your household/family waiting for a COVID-19/SARS-CoV-2 PCR test result?

  • Have you or any member of your household/family had a confirmed diagnosis of COVID-19 in the last 10 days?

  • Please arrive 10 minutes ahead of your appointment time, attending alone where possbile.

  • Please sanitise your hands using the sanitiser available at the front door.

  • Please don a mask and enter the practice, making your way directly to reception.

  • A member of staff will ask you our Covid-19 screening questions, and you may be asked to update your Medical History form. Please ensure that you have details of your current medications in order to complete the update successfully.

  • If we are not ready to take you directly into the surgery, you may wait in the waiting room (maximum of 3 people at any one time to allow for social distancing) or on the forecourt.

  • Once invited into the treatment room you will be asked to re-sanitise your hands and place your personal items in a lidded box before your treatment commences.
