Covid-19 Patient Information

AND ATTENDANCE ADVICE

 

We are screening for respiratory symptoms and Covid-19 prior to your appointment. If you answer ‘Yes’ to any of the following, please call us on 01844 215404 to discuss this with a member of staff before attending for your appointment.

  • Do you have any of the following symptoms:

    • A high temperature or fever?

    • A new, continuous cough?

    • A loss or alteration to taste or smell?

    • A headache?

    • An aching body?

    • Feeling or being sick?

    • A blocked or runny nose?

    • A sore throat?

    • Shortness of breath?

  • Have you or any member of your household had a positive lateral flow or a confirmed diagnosis of Covid-19 in the last 10 days?

 

Attending your appointment during the current stage of the Covid-19 pandemic:

  • Please arrive at least 10 minutes ahead of your appointment time, attending alone where possible.

  • Please sanitise your hands using the sanitiser available at the front door.

  • A member of staff will ask you some screening questions, and you may be asked to update your Medical History form. Please ensure that you have details of your current medications in order to complete the update successfully.

  • Once invited into the treatment room you will be asked to re-sanitise your hands and place your personal items in a lidded box.

  • Please don a mask and enter the practice, making your way directly to reception.

  • A member of staff will ask you our Covid-19 screening questions, and you may be asked to update your Medical History form. Please ensure that you have details of your current medications in order to complete the update successfully.

  • If we are not ready to take you directly into the surgery, you may wait in the waiting room (maximum of 3 people at any one time to allow for social distancing) or on the forecourt.

  • Once invited into the treatment room you will be asked to re-sanitise your hands and place your personal items in a lidded box before your treatment commences.
